Here is an unusual birthday gift idea - "Pay it forward!"
If someone you care for is celebrating his / her birthday and you stuck for a gift idea since they already have all they need and much more, then how about giving them a meaningful present, one that makes a difference?

You can take the same amount of money you were planning to spend on a birthday gift, and make something good with it.
Here are a few ways you can pay it forward:
If your birthday celebrant has a preferred charity organization or cause, then make a donation on their behalf and ask the organization to send you a "thank you" letter with their name on it, so that you can give it to them on their birthday.
If you don't know their preferences, you can donate to aid organizations, to the cancer research fund, animal's shelters, plant trees, or whatever you believe is a justified cause.
Compose a nice birthday card for your birthday celebrant letting him/ her know that you have delivered gifts on their behalf. (Or you can invite him/her to come with you and deliver the gifts together.)
There are many kids that can't celebrate their birthday because of their parents hurdles and the "birthday angels" aim to provide them with their own birthday party.

Consider buying birthday gifts from stores that donate some of their profits to charity, such as the pink ribbon collection items that a percentage from every sale goes to The Komen Foundation in support of its efforts fighting breast cancer.
By paying it forward you get the opportunity to make a difference, to
give back to those who need it the most. You'll be proud of yourself!
Isn't it a truly great birthday gift idea?
